Policies & Billing

We are dedicated to building a successful doctor-patient relationship with you.

As such, your clear understanding of our fee policies is vital to our professional relationship. Our fees are based on our level of expertise, time, staff support, and technologies that we use during your visit to our clinic. Please ask any member of our team if you have questions about our fees and billing policies.

OHIP

OHIP (Ontario Hospital Insurance Plan) will cover the costs of some of our patient’s visits, as outlined by OHIP regulations.

Children up to the age of 19 and seniors 65+ are insured for yearly examinations. Yearly examinations for adults age 20 to 64 with qualifying medical conditions as defined by OHIP regulations are also insured.

Any fees for non-OHIP insured diagnostic/treatment services are the responsibility of the patient.

Payment Options

Cancellation Policy

We kindly ask patients to give 24-hour notice if they are unable to attend an appointment. If a patient misses an appointment without proper notice, a cancellation fee will be charged.

Payment Policy

We make every effort to provide patients with optimal care at a sensible cost, following the fee guides published yearly by the Ontario Association of Optometrists.

For your convenience, we accept payment by Visa, MasterCard, or debit.

The patient is fully responsible for all fees charged by this office, on the day of service, regardless of insurance coverage.

Payments are due at the time services are rendered.

Third Party Billing

If you have insurance coverage for your examination and eyewear, please bring your insurance card to your visit. If we can directly bill your insurance provider, we will be happy to file your claim for payment of services. Any deductible or portion due from the patient will be due at the time of service rendered.

In the event that we cannot directly bill your provider, we will be happy to help you complete your paperwork to file your claim. Payment will be due at the time of service and it is your responsibility to receive reimbursement.
